Your initial consultation is an opportunity to establish care with our provider and discuss your health concerns, medical history, and wellness goals. With shared-decision making, we may assess any existing lab results and determine the need for additional testing to gain a clearer picture of your current health status. This will add to the personalized approach to care by helping to identify underlying conditions and guide the next steps in your treatment plan.
Additionally we may explore factors such as nutrition, physical activity, metabolism, and potential medical support to help you achieve and maintain your health goals. This is intended to provide guidance, support, and evidence-based strategies tailored to your individual needs.
This appointment does not include the lab testing itself or the review of the lab results. That will be part of the follow-up visit.
A follow-up appointment is designed to review your progress and ensure that your care plan is on track. During this visit, we will discuss any changes to your health, evaluate how well your treatment or wellness plan is working, and make adjustments as needed. It’s an opportunity to address new concerns, refine goals, and ensure that your personalized care continues to meet your needs. Whether you're continuing treatment, managing a condition, or enhancing your overall well-being, we are here to support your ongoing health journey. This visit provides ongoing guidance and encouragement to help you achieve long-term success.
With medications, follow-up appointments will be scheduled at a minimum of every three months. More frequent when starting new medications or making medication changes.
During your initial medication management appointment, our healthcare provider will conduct a comprehensive review of your medical history, current medications, and treatment goals. We’ll discuss any concerns, side effects, or adjustments needed to optimize your medication plan for safety and effectiveness. This appointment is intended to provide you with the highest quality care that meets your specific health needs.
At a minimum of every three months. More frequent when starting new medications or making medication changes.
During your follow-up medication management appointment, our provider will assess your response to prescribed medications, address any side effects or concerns, and make any necessary adjustments to optimize your treatment plan. This visit ensures your medications remain effective and aligned with your health goals, providing ongoing support for your well-being.
*Note that medication refills need to be made 72-business hours in advance of your medication running out. Medication refills will not be completed if a follow-up visit has been missed. Please plan your appointments accordingly.
